Sports Physicals
A VHSL Sport Physical is NOT the same thing as an annual physical for school! VHSL Physical must not be dated any earlier than May 1st for the upcoming school year (ie May 1st, 2025 for 2025-2026 school year). A physical dated prior to May 1st cannot carry over to the next school year.
The attached form must be completed in full by the student, parent, and the doctor, and turned in to the School Secretary.
You must have a VHSL form on file if you plan on participating in ANY sport (including tryouts)
If you are a middle school student and you play a sport at the high school level, you MUST get two copies of your VHSL- One for the high school and one for the middle school.
